29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Non-ferrous Building:  Employees were exposed to crushing hazards while trying to repair a forklift where energy control procedures were not developed, documented, or utilized.  An employee was seriously injured when he was pinned between the mast and the overhead guard of the forklift where all sources of hazardous energy were not controlled.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5):  Non-ferrous Building:  Employees were exposed to struck-by and crushing hazards while operating, and working around employees who were operating the Nissan 50 Model J02A24V forklift, where the employer did not ensure that the operators were adequately trained.
