29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. 18 Wilmington West Chester Pike, Chadds Ford, PA - The employer did not inspect excavations prior to letting employees perform work inside excavation thus exposing employees to cave-in hazards. This condition was observed on or about July 5, 2017.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 18 Wilmington West Chester Pike, Chadds Ford, PA 19317 - The employer allowed the use of a portable step ladder that was approximately four feet tall as a means of access into and out of an excavation that was approximately five feet 2 inches deep. The side rails of the ladder were below the upper landing surface, thus exposing employees to fall hazards. This condition was observed on July 5, 2017.
