29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. Materion Brush, Inc. - Elmore, OH: On or about June 23, 2017, the employer did not ensure energy control procedure(s) for the Finish Pickle Line identified specific steps to follow to render the gravitational and pneumatic energy sources safe. The procedure(s) lacked specific steps for blocking and securing the machine's pivot arm and loading table. Employees were exposed to hazards, such as struck-by and pinch points, when servicing the machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Materion Brush, Inc. - Elmore, Ohio: On or about June 23, 2017, employees that entered the Finish Pickle Line exit area were exposed to injury from moving parts of belt-wrapper, loading table, ringer loader table, mandrel, and upper and lower control arms in that the machine was not locked out prior to set-up for a new coil run. The employer did not ensure that energy control application steps to control the electrical/hydraulic, pneumatic, gravitational, and mechanical energy sources were implemented prior to the servicing task - the employer did not: - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, blocked, and rendered safe. - Ensure that the authorized employee verified isolation and deenergization of the machine or equipment had been accomplished.
