29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: A) Materion Brush, Inc., located at 14710 West Portage River South Road, Elmore, OH 43416: On or about August 15, 2018, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the Eitel Stroke Controlled Straightening Press # 1 (Type RP 60, Machine # 25374) prior to set-up activities. Employees conducted set-up activities such as, but not limited to, changing ram heads at the Eitel Press # 1 and were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Materion Brush, Inc., located at 14710 West Portage River South Road, Elmore, OH 43416: On or about August 15, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed prior to set-up activities. Employees conducted set-up activities such as, but not limited to, changing ram heads at the Eitel Stroke Controlled Straightening Press # 1 (Type RP 60, Machine # 25374). Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conducted activities such as, but not limited to, changing ram heads at the Eitel Press # 1. Materion Brush, Inc. was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1910.147(d)), which was contained in OSHA Inspection # 1244701, Citation number 1, Item number 2, and was affirmed as a final order on 12/28/2017, with respect to a workplace located at 14710 West Portage River South Road, Elmore, OH 43416.
