29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  (a) Date of Violation: On or about 07-06-2017 and times prior thereto, located at NAS Oceana - Dam Neck Annex 352 Bullpup Street.  Virginia Beach, VA 23461, Condition: Employee(s) were exposed to scaffolding planks at heights greater than 10 feet high with gaps in excess of 1 inch.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

(a) Date of Violation: On or about 07-06-2017 and times prior thereto, located at NAS Oceana - Dam Neck Annex 352 Bullpup Street.  Virginia Beach, VA 23461, Condition: Employee(s) were only able to climb on and through the cross braces on the tubular welded frame scaffold system in order to access and egress the extended work platform where they were standing while pumping grout in the block walls.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Virginia Beach, VA 23461, Condition: Employee(s) were exposed to excess debris that accumulated on and about the walking and working surfaces of the scaffolding platforms.  Building materials, medium and small sized loose concrete debris/rocks/gravel, loose lumbar, and excess scaffolding parts was found about the entire platform.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    (a) Date of Violation: On or about 07-06-2017 and times prior thereto, located at NAS Oceana - Dam Neck Annex 352 Bullpup Street.  Virginia Beach, VA 23461:    Instance #1:  Employee(s) were exposed to open sides where guardrail systems were not installed along the 90 degree turns around the corners of the structure.  Approximate height to ground below was 15 feet.    Instance #2: Employee(s) were exposed to an inadequate guarded side of the extended work platform where there was not an adequate guard rail system nor was personal fall protection used.  A platform extended side-bracket was placed at the end to act as a guardrail system.  Approximate height to ground below was 19 feet.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
