29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:                                                                                                              The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 19964 Monroe St., Omaha, Nebraska 68136 on or about June 28, 2017. The employer did not ensure that adequate fall protection was provided for and worn by employees at all times while working on the roof of a home which was approximately 12 feet from a lower level.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 19964 Monroe St., Omaha, Nebraska 68136 on or about June 28, 2017. The employer did not ensure that employees were trained to recognize fall hazards.  The employer did not ensure that employees knew to use adequate fall protection when they were performing residential siding work while the roof of a house.      Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.

This was most recently documented at a worksite located at or near 19964 Monroe St., Omaha, Nebraska 68136 on or about June 28, 2017.The employer did not ensure that the portable ladder being used by employees to access the roof extended at least 3 feet above the roof.
