On 7/7/2017, at the Buford facility, employees performing Authorized work at the facility were not required to utilize established Lockout procedures established resulting in an amputation injury.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.     On 7/10/17, at the Buford facility, the employer did not provide Authorized employee training to those employees providing service and maintenance to equipment and machines at the facility, including at the K--Line 2 press.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks.    On 7/7/17, at the Buford facility, the employer did not ensure machine guarding was installed to prevent employee injury. Instances include:    a - The K-Line 2, side 1, Punch Box had no guards on either end of the box. The exposed areas of the press measured 10 inches long x 5 ½ inches     b - K-Line 2, side 1 - geared rollers were not guarded
