29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace, prior to permitting an employee to operate a powered industrial truck:  The employer is failing to protect employees from a struck by hazard when operating an all terrain forklift at a residential construction site.  This was most recently documented at 3241 N 94th St, Lincoln, Nebraska, on or about June 27th, 2017 when an employee was operating an all terrain forklift and he had not received any training.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   The employer is failing to protect employees from fall hazards when conducting framing operations on a residential structure.  This was most recently documented at 3241 N 94th St, Lincoln, NE, on or about June 27th, 2017 when an employee was observed installing tar paper to a residential structure while working greater than 6 feet above the ground without fall protection in use.

This was most recently documented at 3241 N 94th St, Lincoln, NE on or about June 28th, 2017 when an employee was observed installing tar paper on the roof of a residential structure and he had not received fall protection training.
