29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a)1761 Route 9 North, Howell, NJ, 07731- Blending area: Employees performed maintenance, servicing, and general cleaning on a Scott Equipment Company blend mixing machine and the associated electric and pneumatic equipment where the employer did not establish a hazardous energy control procedure, which resulted in an amputation of an employees fingers; observed on or about 6/21/17.  b)1761 Route 9 North, Howell, NJ, 07731- Blending area: Employees performed maintenance, servicing, and general cleaning on a Williams & Wilson blend mixing machine (serial number D- 2666) and the associated electric and pneumatic equipment where the employer did not establish a hazardous energy control procedure, observed on or about 6/21/17.
