29 CFR 1926.501(b)(13): Each employer engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, or personal fall arrest systems: The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of re-roofing operations. Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection. The workers were exposed to falls of over 11 feet to dirt and concrete below. This was most recently documented at 11829 S. 52nd St., Papillion, Nebraska on or about June 26th, 2017 when the employer had employees working on the roof of this residence without any form of protective system. The Flores Construction, L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1165255, citation number 1, item number 2 and was affirmed as a final order on August 11, 2016, with respect to a workplace located at 15014 Sprague St., Omaha, Nebraska 68116.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof. Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection. The workers were exposed to falls of over 11 feet to dirt and concrete below. This was most recently documented at 11829 S. 52nd St., Papillion, Nebraska on or about June 26th, 2017 when the employer had employees working on the roof of this residence without any form of protective system and had not provided any training to them.
