OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees were exposed to an unplanned collapse while working from an improperly maintained truck-mounted aerial ladder: a) CVS Pharmacy, 405 N. Butler St, Sunbury: On or about July 14, 2017, an employee was working from a truck-mounted (GMC 2005) Wilkie (Model 60) aerial ladder to install lights on a sign when the ladder broke off from the bed of the truck at the base, causing the ladder to collapse and the employee receiving serious injuries. The ladder was extended approximately 16 feet 4 inches. Bolts and base plates used to secure the base of the ladder to the bed of the truck were either missing or deteriorated. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS VIOLATION

29 CFR 1910.140(c)(18): Personal fall protection systems were not inspected before initial, use during each workshift for mildew, wear, damage, and other deterioration, and defective components were not removed from service. a) CVS Pharmacy, 405 N. 4th Sunbury, PA: Fall protection being used by an employee to perform work activities while elevated more than 16 ft above the ground level on a Wilkie Model 60 aerial ladder was damaged. The 6 foot lanyard was frayed and was covered in white paint, the safety harness did not have a legible manufacturers label, on or about July 14, 2017. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.
