29 CFR 1910.29(f)(4):  Openings in a stair rail system exceeded 19 inches (48 cm) at its least dimension:    At 9832 Evergreen Industrial Drive, St. Louis, MO 63123, specifically inside of the Maintenance Shop, a metal flight of stairs with one open side, 13 risers high, was not equipped with an intermediate rail.  The opening between the top rail and the stair tread measured approximately 34 inches.  This violation was observed on 07/18/17.

The PVA Station was part of the Retroflex Single-Color Flexo Printer, m/n 20114613, a.k.a. Press #17.  One employee opened up the guard to determine the source of a leak and became exposed to unguarded gears.  A multiple amputation occurred on or about 07/14/17.
