29 CFR 1910.147(d)(2):     The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.    On or about 6/06/2018, equipment in the Print and Roll Stock Department at Packaging Concepts, Inc., located at 9832 Evergreen Industrial Drive in St. Louis, MO, was not de-energized prior to being serviced.  Employees failed to turn off or shut down the equipment during the following instances:    a)  On 6/04/2018, the Wolverine Flexographic Mfg. Co. Press, Model 33-1 / SN# CPN 1/2 -2532 was not turned off or shut down using the established procedures prior to servicing the equipment.  An employee suffered an amputation while attempting to clean the equipment.  b)  On 6/06/2018, Wolverine Flexographic Mfg. Co. Press, Model 33-4 / SN# CICL-2527 was not turned off or shut down using the established procedures prior to servicing the equipment.  An employee was observed standing on the equipment while cleaning the rollers.  The equipment was operating while the employee was servicing/cleaning it.       Packaging Concepts, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1910.147(d)(2) , which was contained in OSHA Inspection Number 1247171, Citation 1, Item 3 (d), issued on 8/04/2017, and became Final Order on 8/22/2017 with respect to a workplace located at 9832 Evergreen Industrial Drive in St. Louis, MO.
