29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:     A) Vertical Package Conveyor, on or about July 10, 2017, employees were on top of a vertical package conveyor to attach lifting cables onto a crane's hook without means of fall protection, exposing employees to a fall hazard of approximately 5 feet.

C) Vertical Package Conveyor Stand fourth floor, on or about July 10, 2017, the ladder access/egress floor opening was not guarded, exposing employees to a 9 to 27 foot fall hazard.

B) Vertical Package Conveyor Stand third floor, on or about July 10, 2017, the chain guardrails across the open sided area were not of adequate height, exposing employees to an 18 foot fall hazard.   C) Vertical Package Conveyor Stand fourth floor, on or about July 10, 2017, the chain guardrails across the open sided area were not of adequate height, exposing employees to a 27 foot fall hazard.

B) At Facility, on or about July 10, 2017, the employer did not report a hospitalization within 24 hours of the incident involving an employee who sustained serious injuries after falling from a ladder and injuring the employee's right leg.
