On May 17, 2017, the employer failed to report to OSHA a workplace related amputation injury to one of their employees.    The employer has failed to report both an amputation to OSHA within 24 hours.  This most recent occurrence of this took place at the facility located 2100 E. Overland, Scottsbluff, NE where on May 17, 2017 the employer has employees operating the Kettner Automatic Palletizer when an employee attempted to remove a chuck of wood from under the roller conveyor resulting in his hand catching in the pinch point of a chain and sprocket injuring his right thumb.      Western Sugar Cooperative was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1186315, citation number 1, item number 1 and was affirmed as a final order on January 17, 2017, with respect to a workplace located at 3020 State Ave, Billings, MT 59107.
