29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:                                                                                                   The employer is failing to protect employees from amputation hazards associated with inadequately guarded chain and sprocket assemblies.  The most recent occurrence of this was found at the facility located at 2100 E. Overland, Scottsbluff, NE where the employer has employees operating the Kettner Automatic Palletizer line with the chain and sprocket assembly on the roller conveyor left unguarded.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:     i.  On May 17, 2017, the employer failed to report to OSHA a workplace related amputation injury to one of their employees.    The employer has failed to report both an amputation to OSHA within 24 hours.  This most recent occurrence of this took place at the facility located 2100 E. Overland, Scottsbluff, NE where on May 17, 2017 the employer has employees operating the Kettner Automatic Palletizer when an employee attempted to remove a chuck of wood from under the roller conveyor resulting in his hand catching in the pinch point of a chain and sprocket injuring his right thumb.      Western Sugar Cooperative was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1186315, citation number 1, item number 1 and was affirmed as a final order on January 17, 2017, with respect to a workplace located at 3020 State Ave, Billings, MT 59107.

29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.     The employer is failing to accurately record the days away from work as they relate to an amputation injury.  The most recent occurrence of this took place at the facility located 2100 E. Overland, Scottsbluff, NE where on May 17, 2017 the employer has an employee suffer an amputation injury while operating the Kettner Automatic Palletizer without recording the injury as a case with days away from work in block H and an Injury in block M1 of the OSHA 300 Log for 2017.
