On or about July 17, 2017, and times prior thereto, in the Production Area, where employees were exposed to struck-by, electrical shock, and caught-in hazards when setting-up, servicing and/or maintaining Viking Nailer, Turbo 505, ID #2 without written procedures to control the release of potentially hazardous energy.  c. 29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.    On or about April 17, 2017, an employee suffered an amputation to the right second finger while operating a But Back Saw and the amputation was not reported with-in 24 hours.
