29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Throughout the facility: On or about June 15, 2017, and at times prior, the employer exposed employees to hazardous energies, caught-in hazards and amputations hazards in that the machine specific lockout/tagout procedures for equipment such as but not limited to the "Line 1, Bowl Lift #1", "Line 1, Bowl Lift #2" and "Line 1 Batch System" which did not adequately outline the procedural steps for placement, removal, transfer and verification of lockout or tagout devices to control all energy sources when employees performed service or maintenance.
