29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways with pan stairs where the treads and/or landings were not filled in with concrete or other material, and the stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan:  a) North building: On July 13, 2017 and at times prior thereto, the controlling employer Headwaters Construction Company did not ensure that its sub-contractors protected their employees while using permanent metal stairways.  Suitable material was  not fitted into the stair pans, leaving them mostly empty and creating a trip hazard.    Note) Corrected during inspection.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stairrail system along each unprotected side or edge:   a) South building: On July 13, 2017 and at times prior thereto the controlling employer Headwaters Construction Company did not ensure that its sub-contractors protected their employees from open sided stairways.  The controlling employer did not ensure stairrails that had been taken down were replaced prior to allowing the stairs to be used by sub-contractors when working in the stairwell, moving materials from one floor to another and using the stairway for access to the upper floors.    Note) Corrected during the inspection.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a) South building: On July 13, 2107 and at times prior thereto the controlling employer Headwaters Construction, Inc., did not ensure its sub-contractors protected their employees when using stairways where no handrails were in place. This exposed the employees to more serious injuries in the event of a trip and fall on the stairs.          Headwaters Construction, Inc., was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1052(c)(1)(i) which was contained in OSHA inspection number 898599, citation number 2, item number 2 and was affirmed as a final order on May 30, 2013, with respect to a workplace located at 2853 North Cucina Ave, Suite 1, Idaho Falls, ID 83401.    Note) Certification of corrective action is required for this item.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) South and north building: On July 13, 2017 and at times prior thereto the controlling employer Headwaters Construction Company allowed construction debris to accumulate in common areas including tops of stairways, stairway landings and in hallways.  This included but was not limited to, drywall debris, scrap wood, and scrap wood with protruding nails.    Note) Certification of corrective action is required for this item.
