29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: A) Hanon Systems USA, LLC, located at 581 Arrowhead Drive, Carey, OH 43316: On or about April 24, 2017, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the Operating Station 205 and the pallet rotator located at the Final Assembly Line. Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic before employees conducted activities such as, but not limited to, cleaning the filter and adjusting screw location at the Operating Station 205, as well as unjamming the pallet rotator. Employees were exposed to struck-by hazards while all hazardous energy sources were not controlled prior to conducting work.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Hanon Systems USA, LLC, located at 581 Arrowhead Drive, Carey, OH 43316: On or about April 24, 2017, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the Operating Station 205 (OP 205) located at the Final Assembly Line, prior to conducting activities such as, but not limited to, cleaning the filter and adjusting screw location at the OP 205. The Operating Station 205 was not isolated from all energy sources such as, but not limited to, electrical prior to engaging in this maintenance activity. Employees were exposed to struck-by hazards while all hazardous energy sources were not controlled prior to conducting work. B) Hanon Systems USA, LLC, located at 581 Arrowhead Drive, Carey, OH 43316: On or about June 5, 2017, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the pallet rotator located at the Final Assembly Line, prior to conducting activities such as, but not limited to, unjamming the pallet rotator. The pallet rotator was not isolated from all energy sources such as, but not limited to, pneumatic prior to engaging in this maintenance activity. Employees were exposed to struck-by hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from struck-by hazards while employees conduct servicing activities such as, but not limited to, adjusting screw location at the Operating Station 205 and unjamming the pallet rotator located at the Final Assembly Line.
