29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly, and maintained in a safe condition: On or about July 06, 2017, employees who access the production area were not protected from potential trip/ fall hazards in that, the floor openings were uncovered. The deficient condition of the flooring includes; but is not limited to, the following areas: a. The floor opening and/or trough located in front of the electrical room in the production area measured approximately 8" inches across (width) and 8ft. in length was unguarded. b. A floor opening located behind product mixer measured approximately 11" inches across (width) and 12 ft. in length was not fully guarded.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about July, 06, 2017, the employer failed to ensure that the open sides of the work platform regularly accessed by employees tasked with completing the service and/or maintenance for the company's electric arc furnace were guarded by a standard guard rail system. Employees who accessed the area were exposed to a potential fall hazard of approximately 7 feet, 7 inches.
