29 CFR 1910.213(h)(1): The upper hood did not  completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that would automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed:     On or before July 18,2017, without receiving prior approval from the manufacturer, the employer modified the original blade guard of the company's (Delta) long arm radial saw, model number 33-411, serial number 149TFL5501AAP. The modifications did not completely enclose the upper portion of the blade, which had a gap of approximately 1.5 inches and the sides of the lower portion of the blade were inadequately guarded in that a gap of approximately 3/4 inch was exposed.
