On or about July 19, 2017, Port of Detroit Terminal, employees were exposed to being struck-by cargo that was being unloaded from the vessel FULDABORG with the Manitowoc crane, model 4100-W, serial number 41543. Two bridle wire rope slings and associated hooks and shackles did not have safety latches on the hooks.

On or about July 19, 2017, Port of Detroit Terminal, employees were exposed to being struck-by cargo that was being unloaded from the vessel FULDABORG with the Manitowoc crane, model 4100-W, #19, serial number 41543. On or about June 19, 2017, Port of Detroit Terminal, employees performing unloading operations from the vessel FULDABORG were exposed a drowning hazard in that portable ladder provided for emergency egress from the water located at the south end of Berth 1 was damaged and not maintained in a safe condition.

29 CFR 1917.45(k)(2): A designated person did not thoroughly inspect all functional components and accessible structural features of each crane or derrick at monthly intervals: On or about July 19, 2017, Port of Detroit Terminal, employees were exposed to being struck-by cargo that was being unloaded from the vessel FULDABORG due to the failure of functional components of the Manitowoc crane #19, model 4100-W, serial number 41543. The crane was not thoroughly inspected by a designated person at monthly intervals. On or about July 19, 2017, Port of Detroit Terminal, employees were exposed to being struck-by cargo that was being unloaded from the vessel FULDABORG due to the failure of functional components of the Manitowoc crane #20, model 4100-W, serial number 41678. The crane was not thoroughly inspected by a designated person at monthly intervals. The Nicholson Terminal and Dock Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1917.45(k)(2), which was contained in OSHA inspection number 949121, citation number 1, item number 5/b and was affirmed as a final order on March 30, 2015, with respect to a workplace located at 4105 West Jefferson Ave, Detroit, Michigan 48209.
