29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: Employees engaged in material handling activities and mandrel change out operations in the B-vent area were exposed to struck-by hazards in that the sling utilized to lift and move the mandrels was not properly inspected and removed from service prior to use.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: Employees engaged in forklift operations in the middle of the warehouse were exposed to shock and fire hazards in that the 480 Volt 3 phase 800 Amp panel was not protected from damage where exposed to forklift traffic.
