29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: In the maintenance area, employees engaged in grinding, and sharpening activities using the Dayton bench grinder were exposed to struck by hazards in that the adjustable tongue guard opening was not maintained at a maximum of one fourth inch. The tongue guard was not installed, leaving a 3/4" gap from the abrasive wheel to the wheel guard.
