On or about July 19, 2017, at the above addressed jobsite, an employee working in a excavation was not wearing a hard hat to protect against falling materials and debris; thereby exposing the employee to head injuries.

On or about July 19, 2017, at the above addressed jobsite, employees were conducting excavation operations along an active roadway with no traffic signs posted, thereby exposing the employees to being struck by vehicular traffic.

On or about July 19, 2017, at the above addressed jobsite, an inspection of the excavation was performed by a competent person and an employee was not removed from the excavation which lacked cave-in protection, thereby exposing employee to the hazards associated with cave-ins.

On or about July 19, 2017, at the above addressed jobsite, an employee was exposing street light wires in an excavation that measured at six (6) feet, six (6) inches in depth.  The excavation was not provided with cave in protection, thereby exposing the employees to cave in hazards.
