29 CFR 1910.22(a)(2): The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided. On or about July 19, 2017, employees in the production area were exposed to slip and fall hazards due to wet floors from the accumulation of water from draining and cleaning the mixing vessels throughout the day. The employer did not ensure the production floor was maintained in a dry condition. There is no abatement certification or documentation required for this item.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. On or about July 19, 2017, employees assigned to work in the production area were lifted approximately 20 feet in a forklift attachment basket to access the top of mixing vessels. The basket was not approved by the manufacturer for use with the forklift. There is no abatement certification or documentation required for this item.
