29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.    On or about July 19, 2017, employees were exposed to trip and fall hazards while scraping rollers on the Tow/Roller Lines due to exposed holes adjacent to the controls for the roller motors.  The holes were less than 4 feet above a lower level and the employer did not ensure covers or guardrail systems were installed to provide a safe walking working surface.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
