29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about July 19, 2017 at the worksite at 202 Lake Valley Drive, Maumelle, Arkansas, an employee walked below an employee who was working overhead without a hard hat. This condition exposed the employee to struck by hazards.

29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On July 19 at the worksite located at 202 Lake Valley Road, an employee was using a pneumatic nail gun without eye protection. This condition exposed the employee to struck by injuries to the eye.

29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from  the work did not automatically and instantly return the lower guard to the  covering position of the saw.    On July 19, 2017 at the worksite located at 202 Lake Valley Drive, Maumelle, Arkansas, a wooden block was placed in the guard of the 7 1/4 inch skilsaw brand circular saw so that the guard was rendered ineffective. This condition exposed employees to amputation hazards.

This condition exposed employees to the hazard of falling to the concrete foundation below.
