29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  On the south side of the construction site, employees working from a tubular welded frame scaffold that was not fully planked on all working levels were exposed to falling hazards.

29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:   On the south side of the construction site, employees working from the second-tier tubular welded frame scaffold without base plates and mud sills installing stucco and metal mesh was exposed to fall hazards of approximately 12 feet to the ground below.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   On the south side of the construction site, the employer did not provide a scaffold ladder where employees could access and egress the tubular welded frame scaffold safely exposing them to fall hazards.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On the south side of the construction site, employees were observed working from the second-tier tubular welded frame scaffold without a fall protective system while performing masonry work. The employees were exposed to a fall hazard of 12 feet to the ground below.

29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toe boards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   On the south side of the construction site, employees working from a tubular welded frame scaffold without toe boards being installed were exposed to falling objects.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On the south side of the construction site, employees used an extension cord to energize a Ridgid air compressor, and the outer sheathing pulled away exposing internal wiring exposing employees to electrical shock hazard.
