29 CFR 1926.703(a)(1): Formwork was not designed, fabricated, erected, supported, braced and maintained so that it would be capable of supporting without failure all vertical and lateral loads that could reasonably be anticipated to be applied to the form a) On or about June 30, 2017; second floor area of construction; the employer had not ensured that the forms used in the process were properly secured/braced to prevent them from displacing and exposing employees to a 16 foot fall to the concrete floor. The form was braced in place by three (3x) legs, around the columns which were not secured at the top or lower levels. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
