29 CFR 1910.37(a)(2): Exit routes must be arranged so that employees will not have to travel toward a high hazard area, unless the path of travel is effectively shielded from the high hazard area by suitable partitions or other physical barriers: The exit route from the rear door contained five, 12 inch diameter square holes approximately 1 to five inches in depth. The path of travel was not shielded from the holes.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: The Emergency Preparedness and Response Plan instructs upon discovery of a fire, if it cannot be easily extinguished to activate the fire alarm and evacuate the building. All staff members were not provided an educational program to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.

(b) At the facility, portable fire extinguishers located in the vehicles were not subjected to annual maintenance checks to include documentation made available to the Assistant Secretary upon request. Extinguishers observed were located in the Chevy pickup license A373150. Badger model B417T . (c) At the facility, portable fire extinguishers located in the vehicles were not subjected to annual maintenance checks to include documentation made available to the Assistant Secretary upon request. Extinguishers observed were located in the Chevy Malibu license A346052. Badger model 250B-1 . The USDA was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3) which was contained in OSHA inspection number 1214522, Notice 2, Item 1 and was affirmed as a final order on June 2, 2017 with respect to a workplace located at 1898 Kountry Lane St. 1, Fort Dodge, IA 50501. The USDA was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3) which was contained in OSHA inspection number 11182315, Notice 1, Item 3 and was affirmed as a final order on 11/17/2016 with respect to a workplace located at 1918 Greene St., Adel, IA 50003.

The USDA-NRCS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.157(f)(2) which was contained in OSHA inspection number 1214522, Notice 2, Item 2 and was affirmed as a final order on June 2, 2017 with respect to a workplace located at 1898 Kountry Lane St. 1, Fort Dodge, IA 50501.
