OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard from falling objects being lifted using a hoist and vacuum lifters. a) By the plasma cutting table, on or about 6/21/17: A hoist attached to a trolley with a rated lifting capacity of 1 ton was used to pick up an Anver VPE1-XD vacuum lifter and a plate of hardox steel with a combined weight of approximately 2399 pounds. The weight lifted exceeded the rated capacity of the hoist and trolley. b) By the plasma cutting table, on or about 7/20/17: Corrective actions were not taken for a finding noted during the inspection of a Coffing hoist, serial #ECE167SPD. The inspection noted that the trolley frame had been welded. c) By the plasma cutting table, on or about 7/20/17: The employer did not conduct periodic inspections of an Anver VPE1-XD vacuum lifter and Anver VPF 57 AC vacuum lifter. d) By the plasma cutting table, on or about 8/17/17: An Anver VPF 57 AC vacuum lift was missing its data plate label and operational controls label. e) By the plasma cutting table, on or about 6/21/17: The Anver 1VPE-XD vacuum lift had the knob removed on the attach/release control button. f) By the plasma cutting table, on or about 8/17/17: The Anver VPF 57 AC vacuum lift was being operated at vacuum pressure levels below manufacturer recommendation for safe vacuum pressure. g) At the facility, on or about 7/20/17: The employer did not maintain user manuals for either the Anver VPF 57 AC vacuum lift or Anver VPE1-XD vacuum lift. Abatement certification must be submitted for these items.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) On the Anver VPE1-XD vacuum lifter, on or about 8/17/2017: Strain relief was broken on the connection of a flexible cord to the control pendant. Wires were sticking out the top opening of the pendant. Abatement certification must be submitted for this item.
