29 CFR 1910.179(n)(2)(ii): The load was not attached to the load block hook by means of slings or other approved devices:  a) Tailgate Area, 365 Upper Oakwood Ave., Elmira, NY, 14903, on or about 7/18/2024; Employees in the tailgate welding area were using a 2-ton Cone Hoist to lift tailgates. Employees were attaching the load to the load block hook with a homemade lifting device.  Abatement certification is required.

29 CFR  1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) 365 Upper Oakwood Ave., Elmira, NY, 14903, on or about 7/18/2024; Employees were required to wear respirators including, but not limited to, 1/2 face elastomeric Miller LPR-100, 1/2 face elastomeric 3M 6200, and Full-face elastomeric 3M 6900, while completing tasks including, but not limited to, welding stainless steel and/or galvanneal, while sanding, and while painting in the spray booth. The employer did not provide employees with a medical evaluation.

Paint Prep Area, Air-Flo Manufacturing, Inc., 365 Upper Oakwood Ave. Elmira, NY 14903, on or about 7/17/2024; Employees were exposed to corrosive chemicals, including, but not limited to, Citric Aluminum Brightener (pH 0.5-2), HTR-120 Solution (pH 1), and E-Nox Clean (pH 0-1) and were not provided with a suitable eyewash for emergency use. Eyewash had accumulation of rust/debris in the water line.  Abatement certification is required.
