29 CFR 1910.22(a)(3): (3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a) WW Drilling, LLC at County Road 12 and County Road RR Anton, CO 80801: On or before July 18, 2017 the employer did not ensure that the rotary table was maintained free of hazards. Manual makeup and breakup tongs were used to make the initial break of a joint. A floor hand was standing near the rotary table in motion to help disconnect the tong from the pipe. The driller placed the rotary table in motion to help disconnect the tong. The floor hand was struck by the makeup tong, resulting in a hospitalization. The employer did not ensure that the rotary table was free from hazards prior to engaging the rotary. Among other methods, one feasible and acceptable abatement method is stated in American Petroleum Institute recommended Practice 54 Recommended Practice for occupational Safety for Oil and gas Well Drilling and Servicing Operations, Section 9.8.1, which provides, The operator shall not engage the power to begin rotation until the rotary table is clear of all personnel and materials.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use: (a) WW Drilling, LLC on Rig # 20 at County Road 12 and County Road RR, Anton, CO 80801: On and before July 20, 2017, the employer did not have a safety data sheet for each hazardous chemical and lubricant, flammable liquid, and combustible liquid stored on location. These chemicals included Argon, Nitrogen, and used oil. This condition exposed employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.
