29 CFR 1910.147(c)(1): Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Injection Molding Area:  On or about July 25, 2017, and at times prior thereto, employees were exposed to the startup of the Engel Victory 1050 injection molding machine while climbing inside to modify the mold mounting platen without locking out all sources of energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a). Maintenance Area:  On or about July 25, 2017, and at times prior thereto, an employee was exposed to a caught in hazard while working with a Jet TPFA-2 mill press that did not have a safety shield or guard in place at the point of operation.      (b). Extrusion & Profile Area:  On or about July 25, 2017, and at times prior thereto, employees were exposed to a caught-in hazard while working near the cutting unit on a Davis-Standard Extruder that was not guarded on all sides.

Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted:  Maintenance Area:  On or about July 25, 2017, and at times prior thereto, employees were exposed to fire hazards as an exit sign was not illuminated in the warehouse.
