29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a) On or about July 25, 2017, the lockout procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and control hazardous energies on the Gamma bag cutting machine. Employees were exposed to caught-in hazards when the pneumatics were not isolated while performing maintenance and servicing.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On July 20, 2017, the employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks were performed on the Gamma bag cutting machine (model # 9418S, serial #2290). An employee suffered an amputation injury when a second employee energized the machine while work was being performed.
