29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. Accutech Films, Inc. - Coldwater, Ohio:  On or about July 18, 2021, the employer failed to ensure the Line 1 winder was guarded to protect employees from rotating parts and nip points.  Employees performing changeover operations were exposed to pinch points created by the rotating turret, rotating winder, and the ingoing nip point created between the plastic film and the winder; an employee sustained fracture injuries to the arm after being caught in the nip point.  b.  Accutech Films, Inc. - Coldwater, Ohio:  On or about July 18, 2021, the employer failed to ensure that the pinch points, created when the chuck is operated to secure the winder mandrel on the Line 1 Winder, were guarded to protect employees from crushing and/or amputation injury.  c. Accutech Films, Inc. - Coldwater, Ohio:  On or about July 30, 2021, the employer failed to ensure that the pinch points, created when the chuck is operated to secure the winder mandrel on the Line 21 Winder, were guarded to protect employees from crushing and/or amputation injury.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
