Safety of equipment was not determined by classification by type, size, voltage, current capacity, and specific use: Employees engaged in, but not limited to, housekeeping activities in the mill on, but not limited to, the steel deck were exposed to fire and explosion hazards in that the electrical system had been de-classified in the following locations due to the following conditions: A: The 480 VAC electrical junction box on the steel deck was missing multiple cover screws. B: The load break elbow, directly above the electrical junction box on the steel deck was missing the panel cover. C: The 2 openings on the side of the electrical junction box on the steel deck was missing the plugs.
