29 CFR 1910.157(e)(3) as required by 29 CFR 1960.8(b): Portable fire extinguisher(s) were not subjected to an annual maintenance check:  The Kiddie Dry Chemical Fire extinguisher No. BK-275935 located in the Chevrolet Colorado, license A351828, had not been subjected to an annual maintenance check.  No inspection tags or records were present.  The fire extinguisher No. J-341165 located in the Dodge Dakota, license A349968 had not been subjected to an annual maintenance check.  No inspection tags or records were present.  The USDA-NRCS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3) which was contained in OSHA inspection number 1182315, Notice number 1, Item 2 and was affirmed as a final order on 11/17/2016  with respect to a workplace located at 1918 Greene St.,  Adel, IA 50003.   The USDA-NRCS was previously cited for a violation of this occupational safety and health standard or its  equivalent standard 29 CFR 1910.157(e)(3) which was contained in OSHA inspection number 1155846, Notice number 1, Item 1 and was affirmed as a final order on 9/1/2016  with respect to a workplace located at 900 4th Ave. Ste., 1, Audobon, IA  50025.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  In the Kitchenette/break room, a receptacle outlet was not provided with ground-fault circuit-interrupter (GFCI) protection for personnel when the outlet was located within approximately four feet of a sink, exposing employees to an electrical hazard.   One feasible Means of Abatement: Install a GFCI to replace the standard outlet or series connect the outlet to the GFCI on the opposite side of the sink in accordance with NFPA 70 - National Electric Code, Article 210.8(B)(5).   The Department of Agriculture-NRCS was previously cited for a violation of this occupational safety and health standard or similar standard 29 CFR 1910.303(b)(1), which was contained in OSHA inspection number 1217889 Notice number 1, Item 1 and was affirmed as a final order on 6/6/2017 with respect to a workplace located at 904 E. Taylor Street Creston, IA 50801.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4:  (a) In the kitchenette/break area, a model 94041 General Electric 15 amp relocatable power tap was furnishing power to two Black and Decker Power coffee pots, exceeding the allowable safe loading capacity of the RPT and receptacle.  The Dept. of Agriculture NRCS was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1215695, Notice number 2, item number 1, with respect to a workplace located in Iowa Falls, IA and became a final order on June 15, 2017.  The Dept. of Agriculture NRCS was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 948245, Notice number 1, item number 2, with respect to a workplace located in Newton, IA and became a final order on January 29, 2014.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).

29 CFR 1910.305(b)(2)(i):  Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    (a) In the NRCS Office, south wall, near the small break area, an outlet did not have a faceplate to protect employees from exposed wiring contacts and damage from insects or rodents entering the box creating a fire hazard.   The USDA-NRCS was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i) which was contained in OSHA inspection number 1182315, Notice number 1, Item 2 and was affirmed as a final order on 11/17/2016  with respect to a workplace located at 1918 Greene St.,  Adel, IA 50003.
