29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Employees performing foundation repair in an excavation approximately 7 feet in depth and approximately 28 feet in length were exposed to trip, fall, and cave in hazards in that a safe means of egress was not provided. Employees climbed the wall of the excavation to exit.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: Employees performing foundation repair in an excavation, approximately 7 feet in depth, were exposed to struck by and cave in hazards in that the spoil pile was not at least 2 feet back from the edge of the excavation.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: Employees performing foundation repair were exposed to trip, fall and cave in hazards in that a the excavation and adjacent areas were not inspected by a competent person prior to the start of the work for evidence of situations that could result in possible cave-ins. The excavation was 7 feet in depth without a protective system, did not have a safe means of egress, and had a spoil pile that was not back at least two feet from the excavation.

Employees performing foundation repair were exposed to cave in hazards in that a protective system was not used while employees worked inside an excavation that was approximately 7 feet in depth, 36 inches wide, and approximately 28 feet in length.
