29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Armando Deanda, doing business as Rhino Roofing on or about July 28, 2017 at the address of 28538 Earl Street, Millbury, Ohio, 43447: The employer did not ensure employees were protected from falls while installing new roofing materials on top of a single story, residential type structure. The employees were exposed to a potential 8.8ft fall to the level below. Armando Deanda doing business as Rhino Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited 1926.501(b)(13), which was contained in OSHA inspection number 1164926, citation number 1, item number 1b and was affirmed as a final order on August 29, 2016, with respect to a workplace located at 630 Wheeling Street, Oregon, Ohio,43616.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Armando Deanda doing business as Rhino Roofing, on or about July 28, 2017 at the address of 28538 Earl Street, Millbury, Ohio, 43447: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. Armando Deanda doing business as Rhino Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited 1926.501(b)(13), which was contained in OSHA inspection number 1164926, citation number 1, item number 1c and was affirmed as a final order, on August 29, 2016 with respect to a workplace located at 630 Wheeling Street, Oregon, Ohio,43616.
