29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level: WAREHOUSE, 555 and 559 NORWICH AVENUE, TAFTVILLE, CT: On or about and at times prior to August 3, 2017, employees working in the storage area (e.g., moving inventory around and sweeping) were exposed to lead-based paint chips that were falling from the ceiling, walls, and other building structures. The employer did not conduct any monitoring or testing to determine the employees' exposure to lead.

29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025: WAREHOUSE, 555 and 559 NORWICH AVENUE, TAFTVILLE, CT: On or about and at times prior to August 3, 2017, the employer had not provided employees working in the warehouse with information pertaining to lead exposure and the associated health hazards. Lead-based paint was peeling from ceilings, walls, and columns and chips were visible on the floors throughout the warehouse area where employees operated a fork truck to move inventory.
