Safety instruction signs were not used where there is a need for general instructions and suggestions relative to safety measures. 1) Room # 23, Ennis , TX facility. On or about June 26, 2017, employees were exposed to herbicide chemical fumes in production room without safety instruction signs for use of personal protective equipment while unloading product from truck located near the herbicide production room.
