29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a) On July 31, 2017, at a jobsite located at 1301 NW Woods Chapel Rd - Blue Springs, MO., employees were exposed to serious injury from potential falls, in that the employer had not trained and instructed workers on how Donn, Doff and use Fall Protection Arrest Harnesses and associated Fall Protection Equipment applicable to tasks required during Metal Decking /Steel Erection Work.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:        (a) On July 31, 2017, at a jobsite located at 1301 NW Woods Chapel Rd - Blue Springs, MO., employees were exposed to serious injury from potential falls, in that employees were using the top guardrails of a scissor lift as a scaffold platform as access to and from the upper metal decking areas of the job site.

29 CFR 1926.454(a)(3): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the proper use of the scaffold, and the proper handling of materials on the scaffold.     (a) On July 31, 2017, at a jobsite located at 1301 NW Woods Chapel Rd - Blue Springs, MO., employees were exposed to serious injury from potential falls, in that employees had not been trained in the proper use and access of a scissor lift when working from elevated work areas.
