29 CFR 1926.502(f)(2)(ii): Warning line systems were not rigged and supported in such a way that its lowest point (including sag) was not less than 34 inches from the walking/working surface: a) On or about August 3, 2017, a warning line system positioned along the edge of the roof deck was not rigged and supported to prevent sagging below 34 inches where employees were exposed to a fall hazard of approximately 35 feet during the performance of roofing work activities. Sections of the warning line system sagged to the roof deck. No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) On or about August 3, 2017, employees were exposed to the hazard of falling approximately 35 feet from the edge of the roof deck while engaged in roofing activities without the use of fall protection or a safety monitoring system. Complete Building Maintenance Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10) which was contained in OSHA inspection number 982278, citation 1, item 2, and was affirmed as a final order on July 29, 2014, with respect to a worksite located at 5304 Hilltop Road, Long Grove, Illinois. No abatement certification or documentation required for this violation.
