29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  (a) At a residential construction site located at 1901 Cemco Dr. in Liberty, MO: Employees were exposed to potentially serious injuries and/or illnesses in that the employer had not developed, or successfully implemented, an effective safety and health program. The employer failed to train their employees on the recognition of hazards associated with falls as was demonstrated on 7/26/2017 when 2 employees were exposed to fall hazards while working from a roof and had not been trained in the regulations applicable to their work.

29 CFR 1926.102(a)(1): The employer should ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) At the residential construction site located at 1901 Cemco Dr., Liberty, MO - Employees were observed working without eye protection while using pneumatic nail gun and hammers without the use of any means of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) At the residential construction site located at 400 Appaloosa Dr. in Raymore, MO:  Employees were exposed to fall hazards while working on a roof without the use of fall protection. This was demonstrated on 6/2/2017 when employees were allowed to work from an approximate distance 9 feet on the eve of the roof to the ground below, and over 16 feet on the highest point to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) At a residential construction site located at 1901 Cemco Dr., Liberty, MO: Employees were exposed to fall hazards. The employer failed to protect employees from hazards associated with falls from ladders. This was demonstrated on 7/27/2017, when employees were using ladder to complete installation of roofing materials in which the employee was extended well outside of the ladder.
