At a new apartment construction site located at 214 Volunteer Ave, Maryville MO -- employees engaged in roofing duties using pneumatic nail guns and other small hand tools were not wearing eye protection. This exposed the employees to getting debris in their eyes causing injury. This was documented on or about 5/29/2024.

The feet of the ladder were not flat on the ground and were folded up so that the ladder's feet were not secure to the ground.
