29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: (a) On or about August 4, 2017 and at times prior, the employer did not ensure that employees required to use nail guns to install roofing material were protected from eye injuries. Employees were not protected with eye protection. MAH Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102 Eye and face protection, which was contained in OSHA inspection number1172580, citation number 1, item number 1 and was affirmed as a final order on September 20, 2016, with respect to a workplace located at 6749 Galley Road, Colorado Springs, CO 80915. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about August 4, 2017 and at times prior, employees of MAH Construction, Inc., were exposed to fall hazards of approximately 15 feet and more while engaged in re-roofing activities without any means of fall protection. MAH Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 Duty to have fall protection, which was contained in OSHA inspection number 1172580, citation number 1, item number 2 and was affirmed as a final order on September 20, 2016, with respect to a workplace located at 6749 Galley Road, Colorado Springs, CO 80915. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
