29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects: At the commercial construction site located at 4024 Noland Rd, Independence, Missouri: Employees engaged in masonry activities on a tubular framed scaffold were exposed to struck by injuries from falling tools and materials. Toe boards were not provided for additional protection.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: At the commercial construction site located at 4024, Independence, Missouri: Employees engaged in masonry activities were exposed to a fall hazard of 23 feet from the upper working level of the scaffold to the ground below. A complete guardrail system or other means of fall protection was not provided. The VISION MASONRY was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(i) which was contained in OSHA inspection number 1050676, citation number 1, item number 2 and was affirmed as a final order on 10/20/2015, with respect to a workplace located at 508 NW Murray Road, Lee's Summit, Missouri.
