Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Chemistry Laboratory - A relocatable power tap (power strip) was connected in series with a Tele-Power Systems relocatable power tap (power strip) which was used to power the computer, monitor, and printer. The power strip was hanging, unsupported, from the Tele-Power Systems power strip creating strain on the insulation exposing employees to electrical shock hazards on or about 8/22/2017. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Laboratory - Employees were not provided with training on hazardous chemicals in their work area at the time of their initial assignment on or about 8/22/2017. b) Chemistry Laboratory - Employees in the Chemistry laboratory were not provided with effective information and training on hazardous chemicals in their work areas as employees were unaware of the hazardous chemicals they were working with including, but not limited to SmpCln1, ISE Cleaning Solution, and Cleancell Elecsys (corrosives), Cavi wipes and alcohol pads (flammable irritants) on or about 8/22/2017. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
